Copy of the page that has the expiring date in your passport.
Copy of recent police records.
Copy of recent criminal records.
Copy of your STCW certificates if you already have them.
Copy of your Seaman's book if you already have one.
Copy of your medicals if you already have them.
Copy of C1/D visa if you already have them.
